These must be reported using the ODJFS Group Provider Information Form 6777 which may be found at http://www.odjfs.state.oh.us/forms/inter.asp Once complete, the form must be mailed to: ODJFS Provider Enrollment Unit, PO Box 1461, Columbus, Ohio 43216-1461 or emailed as an attachment to medicaid_provider_update@jfs.ohio.gov Please include in the email subject line “Form 6777 – Group Practice Information.”
